4. Letter from Dr. Faye C. Marcojos, MD, MHA addressed to the Honorable Members, 21st Sangguniang Panlalawigan, thru Hon. Anthony Joseph P. Cañedo, Sangguniang Panlalawigan Member, all of this Province dated July 30, 2025 in reference to SP Resolution No. 18-25, Series of 2025, earnestly requesting the Office to furnish the Body a copy of the incident report pertaining to the death of Mr. Jovelito Maceda Veras and SP Resolution No. 19-25, Series of 2025, requesting a copy of the hospital policies currently being implemented by Hinatuan District Hospital. She informed the Body that in faithful adherence to the principles of transparency, accountability, and public interest, she is transmitting the following documents: 1. Certified True Copy of the Incident Report regarding the case of Mr. Jovelito Maceda Veras, as recorded by the medical and administrative units, including the sequence of events, personnel involved, interventions rendered, and pertinent clinical findings. The report is submitted with due regard to the confidentiality of patient information and is furnished solely for official legislative oversight purposes; 2. Compendium of Hospital Policies, currently in effect and implemented within Hinatuan District Hospital. Said compendium includes: Anti-Hospital Deposit Law Compliance Frame Work and Policies and Procedures on the Use of Hospital Ambulance, for the Body’s information and appropriate action.

Ordinance No. 68 dated June 24, 2025 of the Municipality of San Agustin, this Province entitled “An Ordinance Creating the San Agustin People’s Council of the Civil Society Organizations of the Local Government Unit of San Agustin for the Institutionalization of a People-Government Partnership for the Formulation of a Comprehensive, Multi Sectoral Development, and Providing Funds Therefor”.
